Submit a Claim - Claim Instructions

To qualify for a payment, you must submit a timely and valid Claim Form with the required supporting documentation. The Claim Form must be submitted by email such that it is received no later than September 22, 2020, or, if mailed, be postmarked no later than September 22, 2020.

You may submit the Claim Form:
